Follow these simple steps to gather your benefit information:
Step 1: Locate Your Insurance Information
Gather the following details from your insurance card:
- Insurance Company Name (e.g., UnitedHealthcare, Regence, etc.)
- Member ID Number
- Group Number (if applicable)
- Customer Service Phone Number (typically on the back of the card)
Step 2: Call the Customer Service Number
- Dial the Customer Service or Member Services number on your insurance card.
- When prompted, choose the option for benefits and coverage.
Step 3: Ask the Right Questions
When you speak with a representative, ask these specific questions:
- Does my plan cover massage therapy?
- Is massage therapy covered as a stand-alone service, or does it need to be part of a physical therapy or chiropractic treatment plan?
- Do I need a referral from my primary care provider (PCP)?
- Is prior authorization required before receiving massage therapy?
- How many sessions are covered per year?
- What is my copay, coinsurance, or deductible for massage therapy?
